How you apply for a Co-operative Funeralcare Funeral Plan depends on whether you have chosen a Set Funeral Plan or a Tailor-made Funeral Plan.
Set Funeral Plans
Apply by phone with one single payment.
Simply call us on 0800 088 4872* - we can complete your application and take your credit or debit card payment over the phone (Please note Direct Debit not currently available).

Apply by post
- Step 1 - print off and fill in a copy of our Funeral Plan Application Form. You will need Adobe Acrobat to open the PDF form, or request a copy to be sent to you. Download Adobe Acrobat here. .
- Step 2 - read our Terms and Conditions attached to the Application Form before you sign it. If you want to pay over 12, 24 or 36 months, or by Fixed Monthly Payments, please make sure you fill in the Direct Debit Instruction.
- Step 3 - send the Application Form back to us at the Freepost address below, making sure you have filled in the payment instructions or enclosed a cheque.
