2. Take the form into your Co-operative Pharmacy, hand it to a member of staff who can explain how the service will work for you.

You can order your repeat prescriptions with us or your doctor:

• Pop into the pharmacy or call us to say which items you need ordering (5 days before you need your repeat medication)

• If you'd like to order your own medication and have us collect the prescription, just call us on the day you order it from the doctors.

3. Collect your medication from your local Co-operative Pharmacy 

We’ll collect your prescription from the doctor's surgery for you and your medication will be ready and waiting for you at the Pharmacy at a convenient time.
